Increasing the size of the text
If you find the text hard to read, move your mouse to the top of your browser window and click on View. On the resulting View menu, move your mouse down to Text Size. Another menu will open to the right, allowing you to click on Larger or Largest.
Going back to a previous page
At the top left corner of your browser you will see a Back button. This button makes it very easy to retrace your steps through the pages you have viewed. Each time you click the Back button, you will go back one page.
Searching the site for information
This site features two search boxes:

At the top of each page is a "-- search PA --" box that will search all Commonwealth of Pennsylvania websites for the term or phrase you enter into the box

In the left-hand gold navigation bar on each page is a "-- search SERS --" box that will search the SERS website for the term or phrase you enter into the box.
Viewing Adobe PDF Documents
All forms on this website are available in PDF format only, best viewed with Adobe Acrobat Reader version 7. If you would like to view and print (where applicable) these forms and you do not have a copy of Acrobat Reader you can get the latest version at no cost from
Adobe's website.
Forms which have the word "Sample" written across them are for informational purposes only. The actual form is available from your local
Regional Counseling Center.
